"The mob" appears to refer to more than just one protestor, so the context that the US embassy is one block away is in fact needed — or else OP would have been more specific with their claim. [Link]

The spider-man costumed person climbs lampposts and scaffolds along the protest route, and Mt. Sinai has a scaffold in the picture. Mt Sinai is also near the US embassy in Toronto. [Link] [Link]

No note needed. Person is indeed climbing and protesting on and in front of the hospital, as the tweet claims. Proposed CNs are speculating that the person didn’t intentionally target the hospital, when everyone has a choice where to protest (and where they climb).
